Care Management
A care management consultation regarding a detailed care plan analysis for your loved one, can be offered by our licensed nurse or gerontologist. Complimentary care management includes weekly updates and management of our caregiver from our staffing director, monthly visits to the home if needed and quality care calls to our staff. Complimentary assessments include a safety check of home environment, medication set up analysis, referrals to in-home geriatricians, pharmacies, home health, hospice and more. Additionally, an activities of daily living assessment and care management suggestions such as a daily care schedule for the caregiver and client as well as an introduction to our technology plan and care plan contract
